Whether you are growing your family or going out on disability, this part of the site will provide you with the options you may need to consider as you go through life's transitions.

Birth or Adoption of a Child

When you are adding to your family, it can be a very stressful yet joyous time.  Please carefully review this section and determine if you need to make changes.

You may not be prepared for possible payroll adjustments.

Check your available PTO and speak with your immediate supervisor about scheduling around the birth or adoption of your child.

Your supervisor, co-workers or client may not be prepared for your absence.

Plan time away from the office accordingly.

You may be eligible for FMLA (Family Medical Leave Act). Discuss this option with your immediate supervisor.

If you don’t use FMLA now, you may not be eligible later. FMLA must be completed within the first 12 months of the child’s arrival.

Plan time away from the office accordingly.

Consider enrolling your child in medical, dental and vision within the first 30 days of birth or adoption.

If you don’t, you may not be eligible and may sustain liability for the cost of the birth and other related expenses traditionally covered under your health plan.

Complete the Benefits Election Form in the Forms Library on the KNOW.  You only need to complete the sections you would like to change and leave everything else blank to continue your current elections.  The effective date of the change will be the date of birth of the child or the date of placement for adoption.  You will also need to provide documentation of the birth or adoption (i.e. copy of hospital letter that verifies date of birth, adoption paperwork).  The Benefits Election Form and documentation must be received in the Benefits Dept. within 30 days of the birth or adoption. 

Consider enrolling in or increasing your election for the Health Care and Dependent Care Flexible Spending Accounts within 30 days of the birth or adoption.

If you do not complete your changes within 30 days, you will not be allowed to make changes until the next open enrollment period.

The Benefits Election Form will have a section for these benefits to be completed if you wish to make a change.

Consider enrolling or increasing your optional life insurance level. You will need to prove insurability.

Your existing coverage will remain in place.

Enter your new election on the Benefits Election Form and the Benefits Dept. will send you the necessary insurance paperwork.

Consider enrolling your child in dependent child life insurance within the first 30 days of birth or adoption.

You will have to add coverage at open enrollment.

The Benefits Election Form will have a section for this benefit to be completed if you wish to make a change.

 

Review your beneficiaries for Basic Life, Supplemental Life, Basic AD&D, Supplemental AD&D and the Retirement Plan.

Your existing beneficiary elections will remain in place.

If you want to make changes to your life and AD&D beneficiaries, login to Employee Self Service through the KNOW and make those changes at any time.  Login to http://www.netbenefits.com/ for Fidelity and http://www.tiaa-cref.org/ for TIAA-CREF to change your beneficiaries for the 403(b) and 401(a) Retirement Plans.


Marriage

While this is a joyous occasion is it usually riddled with chores and long to do lists.  Please carefully review this section and determine if you need to make changes.

If you change your name, update with the Social Security Administration and Noblis HR.

Your tax information will not be correct and may cause problems when you file your annual return.

Call the Social Security Administration at 800-772-1213 and login to Employee Self Service through the KNOW to enter your new name.

If you change your name contact the Security Department.

Your security clearance may be affected.

Enrolling your spouse in medical, dental and vision coverage within the first 30 days of the date of marriage.

You will not be able to enroll your spouse until the next annual open enrollment.

Complete the Benefits Election Form in the Forms Library on the KNOW.  You only need to complete the sections you would like to change and leave everything else blank to continue your current elections.  You will also need to provide documentation of the marriage (i.e. copy of the marriage certificate).  The effective date of the change will be the date the form and documentation is received by the Benefits Department.  The Benefits Election Form and documentation must be received in the Benefits Dept. within 30 days of the date of marriage.

If your spouse has eligible dependents, consider enrolling them in medical, dental and vision.

If you don’t then you will have to wait until the next annual open enrollment.

Add this information onto the Benefits Election Form.

Consider enrolling or changing elections in your flexible spending accounts within the first 30 days of marriage.

If you don’t complete changes within 30 days of your qualifying event, then you will have to wait until the next annual open enrollment period.

The Benefits Election Form will have a section for these benefits to be completed if you wish to make a change

Consider enrolling in or increasing your optional life insurance coverage.

Your existing coverage will remain in place. If you elect to make changes, then you will need to provide proof of good health.

Enter your new election on the Benefits Election Form and the Benefits Dept. will send you the necessary insurance paperwork.

Consider enrolling your new spouse in dependent spouse life insurance within the first 30 days of marriage.

If you don’t complete changes within 30 days of your qualifying event, then you will have to wait until the next annual open enrollment period.

Enter your new election on the Benefits Election Form and the Benefits Dept. will send you the necessary insurance paperwork.  Your spouse will need to complete the Hartford Evidence of Insurability paperwork for elections that exceed $25,000.

Review your beneficiaries for Basic Life, Supplemental Life, Basic AD&D, Supplemental AD&D and the Retirement Plan.

You can make changes to your beneficiaries at any time.

If you want to make changes to your life and AD&D beneficiaries, login to Employee Self Service through the KNOW and make those changes at any time.  Login to http://www.netbenefits.com/ for Fidelity and http://www.tiaa-cref.org/ for TIAA-CREF to change your beneficiaries for the 403(b) and 401(a) Retirement Plans.

If you change any of your bank account numbers for your checking or savings, notify payroll immediately.

Your paycheck will continue to be deposited into your former account until the change is made.

Complete a new Direct Deposit Form and send it to the Payroll Department. You can access the Direct Deposit form through the KNOW in the Forms Library.

Whether you are buying, renting or moving to a new community, there are some important details that you should consider below:

If you need time off from work for packing and moving, remember to request PTO early.

Your supervisor may not be able to approve the time away due to work load responsibilities.

Plan your time away from the office accordingly.

Update your new address in Employee Self Service.

 

Your old address will remain in the system and all correspondence from the company and our vendors will be sent to the incorrect address.

Update your address in Employee Self Service through the KNOW.

If your current medical plan is also available in your new location, consider if you will need to change your current providers.

The distance from your medical providers to your new home may not be a distance that you are willing to travel when you are sick.

Log on to http://www.mycigna.com/ to find new providers in your area.

If you are enrolled in the Cigna HMO and the plan is not available in your new location then contact the Benefits team so you can make changes to your benefits elections.

If you are out of the service area, you may have limited coverage.

If you change any of your bank account numbers, notify payroll immediately if you have direct deposit.

Dropping Dependents

Having teenagers can usually mean anticipating a graduation, or a student turning 19 but attending classes on a part time status, or possibly no longer in school

It is important to know that coverage for a dependent ends as of the end of the month of a child’s 19th birthday if he/she is not a full time student. Dependent coverage ends at the end of the month of a child's 25th birthday regardless of student status.

Notify Benefits to discontinue your child’s participation in the medical, dental, vision and dependent child life plans within 30 days.

You will be paying for the benefit, but claims will be denied if your child is ineligible for the benefit. Your child may miss the COBRA extension period.

Notify benefits@noblis.org to make the changes to your benefits and initiate a COBRA package being mailed.

COBRA Continuation Coverage

Your child may miss their COBRA extension window and lose the right to continue coverage.

COBRA can last for up to 36 months when a dependent loses eligibility under the plans.


Death of a Family Member

While this is always a sensitive time, please know that it may require important changes to your benefits.

Review the Bereavement Policy and speak to your supervisor.

Your absence may be applied to PTO, or unpaid LOA.

Review the Policy 209 in the Policies and Procedures Handbook located on the KNOW.

Consider taking PTO and speak to your supervisor if your absence will exceed the bereavement policy timeframe.

Your absence may be unpaid.

Login to your timecard and check your PTO balance in the upper right corner.

Consider contacting the Employee Assistance Program. You or any other member of your immediate family will have access to this free service.

If you don’t call, you may pass up a valuable counseling service that may help you though this difficult time.

Contact the EAP at 800-607-1522. All conversations are confidential.

File an insurance claim if the family member was covered under Dependent Life Insurance.

You will not receive a death benefit for a covered dependent.

Contact the Benefits Department at benefits@noblis.org.

Discontinue the family member’s participation in the Medical, Dental, Vision and Dependent Life plans within 30 days of the death.

You could be paying extra premiums.

Make any appropriate changes to your Flexible Spending Accounts within 30 days of the death.

You will continue at the current contribution until the next annual Open Enrollment.

The Benefits Election Form will have a section for these benefits to be completed if you wish to make a change.

 

Review the life insurance needed on yourself within 30 days.

Your current coverage will stay in place.

Enter your new election on the Benefits Election Form and the Benefits Dept. will send you the necessary insurance paperwork.

 

Review your beneficiaries for Basic Life, Supplemental Life, Basic AD&D, Supplemental AD&D and the Retirement Plan.

 

No changes will be made to your beneficiaries.  All previous elections will remain in place.

Divorce or Legal Separation

Please review the important decisions and action needed during this life change.

Notify the Benefits team within 30 days of your divorce or legal separation to discontinue your spouse’s participation in Medical, Dental, Vision and Dependent Life Insurance.

Determine if your children/stepchildren are still eligible and if they will continue to be covered by you.

You have 30 days to make changes to your benefits otherwise you will have to wait until the next annual open enrollment period.

If you have direct deposit, and change any of your bank account numbers, then please contact the payroll department.

Your payroll check will continue to be deposited in the former account prior to the divorce.

Complete a new Direct Deposit Form and send it to the Payroll Department. You can access the Direct Deposit form on the KNOW in the Forms Library.

If a Qualified Domestic Relations Order will be part of your divorce, please contact Fidelity for assistance.

Lack of information can result in higher attorney’s fees.

Use Fidelity's QDRO website at http://qdro.fidelity.com/.

If a Qualified Medical Child Support Order will be part of your divorce settlement, you will need to provide a copy of the order within 30 days to the Benefits team.

You will not be able to add your dependents until the next annual open enrollment.

If you change your name, update with the Social Security Administration and Noblis.

Disability

The term disability is defined as being unable to perform the material duties of your job due to a sickness or injury. Secondly, you must be receiving appropriate care and treatment and complying with the requirements of such treatments. Please review this section carefully and determine if any action is needed on your part.

The Benefits team will mail documents to your home address for you and your physician to complete and return. Your disability benefits may be delayed. Complete and return all documents to the Nurse as soon as possible.
Cooperate with the medical certification process in a timely manner. Your disability benefits may be delayed. Work closely with your physician and the Nurse during this process.
Provide the Nurse with a return to work form prior to returning to work. Your return to work could be delayed. Fax the form to 703-610-2240.

A change in work status is a qualifying event and you should be able to join your spouse’s plan if available. You will have to pursue other options for coverage. Contact your spouse’s HR Department within 30 days.
A COBRA package will be mailed directly to your home for you to review and or complete. You will not have the option to continue your current benefits if you miss the 60 day decision window. Review the package that will be automatically mailed to your home address.
Review your home address in Employee Self Service. Your package could be mailed to the incorrect address, thus delaying your enrollment. Change your address in Employee Self Service through the KNOW.


Change in Spouse Employment

A change in your spouse’s employment is considered a qualifying event and you may need to make changes to your benefits. You may have double coverage or no coverage at all if you fail to act within 30 days. Review the benefits information located on Current Benefits page.
